Seirrah OT can provide reports for multiple purposes; including, gaining a second opinion, for personal use, for schools, to demonstrate intervention and progress, following the administering of standardised assessments and to support the educational needs of a child.
Comprehensive reports are available upon request and will be personally tailored to meet the needs of the individual, and meet the purpose it was requested for.
We are often asked to provide reports for the following reasons:
- Create a baseline from assessments and identify the needs of an individual
- Track progress from interventions
- Provide recommendations of equipment or adaptations
- Summarise the assessment completed
- To start a block of therapy sessions
- For education planning and provision purposes
- Annual review
As part of our Occupational Therapy assessments we will provide a report outlining the findings and recommendations. This report is brief and is needed to begin a block of therapy sessions.
Upon request, we are able to produce an in depth report from the assessment which can be used for multiple purposes.
